Implants are used as ‘artificial tooth roots’ to replace missing teeth without having to touch the surrounding teeth. They are placed in the bone and allowed to heal and then crowns or bridges are permanently fixed on the top. They can be used for any situation from a single tooth to a complete set of teeth.

Does it hurt?

There are Five stages of placing an implant

	Stage 1. Evaluation Each patient undergoes a thorough clinical evaluation to determine whether dental implants are possible. In most cases there are no absolute contra-indications to treatment, however risk factors such as smoking, uncontrolled diabetes and post irradiation therapy for cancers are causes for concern. An evaluation will involve measurement and X-rays to determine the amount of available bone into which the implant will be placed. Due to the nature of bone resorption after tooth extraction early placement of an implant is preferable. Diagnostic wax ups may be required that enable the final tooth restoration to be visualized and the determination of the optimal position of the implant.

	Stage 2. Implant placement Gentle surgical techniques are employed to prepare the bone and place the implant. The procedure is generally performed under local anaesthesia or with IV sedation to alleviate any apprehension. There is little postoperative pain or bruising. Once placed, osseointegration occurs over a period of between two to six months. At no time is the patient without teeth unless they elect to do so.

	Stage 3. Connection to the implant. If one views the implant as a replacement for the missing root then an abutment is an extension of this root that comes through the gum allowing the restoration to be built upon this. The abutment is firmly attached to the underlying implant using a predetermined force giving long term stability to the restoration.

	Stage 4. Fixing the artificial teeth. From single to multiple teeth, aesthetic functional restorations can be bonded in place. These look and feel like natural teeth giving a confidence unequalled in dentistry. Alternatively in the case of a patient missing all teeth, but unable for some reason to have as many implants, then a removable denture can be held in place using studs or a clip and bar that allows much increased retention and chewing efficiency when compared to conventional full dentures.

	Stage 5. Follow up All implants require careful maintenance and regular check-ups although with professional instruction their care can be easily incorporated into a daily oral hygiene regime.
